Gross specimen of a nodular goiter. The thyroid gland is bilaterally enlarged by multiple nodules that have a gelatinous, glistening cut surface due to abundant colloid. Markedly enlarged multinodular goiters can produce pressure on trachea and esophagus. Some goiters can grow into thoracic cavity ("intrathoracic" or "plunging" goiter).

This is a characteristic microscopic appearance of a goiter: variable-sized, large follicles filled with pink, watery colloid and lined by flat, inactive follicular cells. Note in the left side the presence of hemosiderin-laden macrophages (brown pigment), indicative of previous hemorrhage. This is a common finding in goiter and leads to cystic degeneration.
